All active Holy Name of Jesus Altar Servers are invited to join us for a fun summer picnic at Hersheypark
We are planning a fun day together at the park, with food, rides and fellowship on Thursday, August 13, and you are invited! We will depart at 9:30 am from Holy Name of Jesus parking lot and return around 7:30 pm. The trip will include lunch together at the picnic pavilion and entrance to the park.
This trip is being offered to all our active Altar Servers for $15.00.
Reserve your spot today, deadline to register is Friday, July 31.
Please return the permission slip that is located below and a check made payable to Holy Name of Jesus Church with a notation “Hersheypark” to the Church office or drop in the weekly collection.

CONSENT AND WAIVER
I hereby give permission for my child, ________________________ , to participate in the event described above. I understand that this event will take place away from the parish grounds and that my participant will be under the supervision of the designated parish/organization employee on the stated dates. I further consent to the conditions stated above on participation in this event, including the method of transportation. I hereby agree, on behalf of the named student and his/her other parent or legal guardians, to waive any claims for liability against this group/parish, the Diocese of Harrisburg (and any diocesan or parish officers, agents or employees) which may arise from the participation of the named participant in the above-described event.
